- This promotion is made between Every Opportunity support Services (EOSS) and said, NDIS participant.
- Limited to one claim of ‘2 hours free support service’ per client.
- Promotion expires 6 months after the date of claim.
- This service amount cannot be exchanged for a different service provided by Every Opportunity Support Services.
- You are obligated to provide EOSS with the necessary documents to complete the service before services commence.
- This promotion can be terminated without notice, if the participant or participant representative treats EOSS staff in a disrespectful and or harmful manner or services are not within a support workers’ rights and or responsibilities.
- For service to commence EOSS must be supplied with a signed 2 hr (no lock in) Service Agreement and completed EOSS Client Information Form.
